Photocopy of Bank or Credit Card Statement, not older than 3 months quoting full name and current address

Photocopy of Utilities Bill i.e. Gas, Water, Electricity, Rates or Television Licence, not older than 3 months quoting full name and current address. Mobile phone bill is not acceptable

Photocopy of Inland Revenue Tax notification
